หนังสือสัญญาฉบับนี้ทำขึ้นระหว่าง
นาง CAROL อยู่บ้านเลขที่ 24/92 หมู่ที่ 6 ตำบลเชิงทะเล อำเภอถลาง จังหวัดภูเก็ต

ข้อ 3. ค่าฤชาธรรมเนียมในการยื่นคำร้องต่อศาล ทนายความจะเป็นชำระเองทั้งหมด
ข้อ 4.ผู้ว่าจ้างจะไม่ถอนทนายความ หากมีการถอนทนายความในระหว่างการพิจารณาคดีของศาล ผู้ว่าจ้างยอมให้ถือว่าทนายความได้ดำเนินคดีถึงที่สุดแล้ว และยอมชำระค่าทนายความที่ค้างอยู่ทั้งหมดทันที
สัญญาฉบับนี้ทำขึ้นสองฉบับมีข้อความถูกต้องตรงกันทุกประการ ทั้งสองฝ่ายได้อ่านและเข้าใจข้อความในสัญญาโดยตลอดแล้ว เห็นว่าถูกต้องตรงกับเจตนาของตน จึงได้ลงลายมือชื่อไว้เป็นสำคัญ
