Heparin lock หรือ ที่ในปัจจุบันนิยม

Heparin lock หรือ ที่ในปัจจุบันนิยมใช้เป็น normal saline lock แทนนั้น เป็นชุดอุปกรณ์ขนาดเล็กที่ใส่คาหลอดเลือดดำไว้สำหรับให้ยาทางหลอดเลือดดำเป็นระยะ ๆ ประกอบด้วย 2 ส่วน คือ ส่วนที่เป็นเข็มพลาสติค (medicut หรือ iv cath หรือ intravenous catheter)ใส่คาไว้ในหลอดเลือดดำ อีกส่วนเป็นรูปทรงกระบอกปลายข้างหนึ่งต่อกับเข็ม อีกปลายเป็นจุกยางปิดไว้เพื่อใช้สำหรับแทงเข็มเพื่อฉีดยา (medication port หรือ injection plug) หรือ อาจใช้สาย extention tub ขนาดเล็กและสั้นแทนจุกยางดังกล่าว

ในชุด heparin lock จะใส่ heparin soulution 10 – 100 unit/cc. เมื่อผสมเสร็จแล้วจะแบ่งใช้ครั้งละ 1.0 - 2.0 cc

หรือ normal saline solution (NSS) ครั้งละ 1-3 cc ฉีด lock ไม่ให้เลือดแข็งตัว หลังเสร็จสิ้นการฉีดยา

วิธีการฉีดยาเข้าหลอดเลือดดำโดยผ่านชุด heparin lock มีดังนี้

1. เตรียมกระบอกฉีดยาบรรจุ heparin ตามวิธีผสมข้างต้นจำนวน 2 cc (ไล่อากาศออกจนหมด )
2. เตรียมกระบอกฉีดยาบรรจุ NSS จำนวน 2 cc (ไล่อากาศออกจนหมด )
3. ทำความสะอาด injection plug ด้วย 70 % alcohol ปล่อยให้แห้ง
4. ฉีด NSS เข้า injection plug 1 cc. เพื่อไล่ heparin เก่า และทดสอบว่า clot หรือไม่ ถ้าฉีดยาก ฝืด ผิดปกติ ห้ามใช้แรงดันฉีดเข้าไป ให้สงสัยว่าเกิด clot ให้ off แล้วแทงใหม่
5. ถ้าไม่มีปัญหา ให้ ฉีดยาเข้าทาง injection plug จนหมดตามขนาดที่ผู้ป่วยต้องได้รับ
6. ฉีด NSS เข้า injection plug 1 cc. เพื่อไล่ยา
7. แล้วฉีด heparin เข้า injection plug เพื่อ lock
วิธีการฉีดยาเข้าหลอดเลือดดำโดยผ่านชุด normal saline lock มีดังนี้

1. เตรียมกระบอกฉีดยาบรรจุ NSS 3 cc. ไล่อากาศออกจนหมด
2. ทำความสะอาด injection plug ด้วย 70 % alcohol ปล่อยให้แห้ง
3. ฉีด NSS เข้า injection plug 1 cc. เพื่อทดสอบว่า clot หรือไม่ ถ้าฉีดยาก ฝืด ผิดปกติ ห้ามใช้แรงดันฉีดเข้าไป ให้สงสัยว่าเกิด clot ให้ off แล้วแทงใหม่
4. ถ้าไม่มีปัญหา ให้ ฉีดยาเข้าทาง injection plug จนหมดตามขนาดที่ผู้ป่วยต้องได้รับ
5. ฉีด NSS เข้า injection plug 2 cc. เพื่อไล่ยา และ lock
กรณีที่ใช้ normal saline lock
ถ้ามีการฉีดยาห่างกันเกิน 6-8 ชั่วโมง อาจทำให้ clot ได้ จะต้องฉีด NSS ทุก 6-8 ชั่วโมง (ทั้งนี้ขึ้นอยู่กับว่าเรา assessment แล้วพบว่า ผู้ป่วยมีภาวะเสี่ยงต่อการ clot ง่าย อาจจะต้อง flush หรือ ฉีด NSS ทุก 6 ชั่วโมง แต่ถ้าผู้ป่วยปกติก็ฉีด ทุก 8 ชั่วโมงได้ โดยควร run เวลาฉีด NSS ใน medication record ไว้เพื่อกันลืมและเป็นหลักฐานว่าพยาบาลได้ให้การดูแลในเรื่องนี้เป็นอย่างดี
