จัดตั้งเป็นอุทยานแห่งชาติโดยมีชื่อว่าอุทยานแห่งชาติมุกดาหาร ตามประกาศในราชกิจจานุเบกษาเล่ม 105ตอนที่ 235ลงวันที่ 28ธันวาคม 2531นับเป็นอุทยานแห่งชาติลำดับที่ 59ของประเทศต่อมาเมื่อวันที่ 31พฤษภาคม พ.ศ. 2549กรมอุทยานแห่งชาติ สัตว์ป่า และพันธุ์พืช ได้พิจารณาชื่ออุทยานแห่งชาติให้มีความเหมาะสม สอดคล้องกับจุดเด่น และศักยภาพที่สำคัญของพื้นที่ที่จัดตั้งเป็นอุทยานแห่งชาติ จึงเปลี่ยนชื่ออุทยานแห่งชาติมุกดาหาร เป็นอุทยานแห่งชาติภูผาเทิบ
