HISTORY: A case presented with bilateral sciatica.
TECHNIQUE AND SEQUENCES: Sagittal: T1W, T2W with and without FS
Axial: T1W, T2W with and without FS
Coronal: T1W
Sagittal and coronal MR myelography
FINDINGS:
The study shows mild scoliosis of lumbar spine. There is grade I L4/5 spondylolisthesis. Few small round hyperintense lesions on T1W/T2W scattering at T12-L2 vertebral bodies are seen, which can be suppressed by FS. The rest of the visualized spine shows normal in height and signal intensity.
There is decreased signal intensity on T2W of the lumbar intervertebral discs, degenerative change. Details of each intervertebral level are as follows;
L2/3 and L3/4 levels: Circumferential disc bulging with hypertrophic facet joints is noted. No significant evidence of nerve root compression is seen.
L4/5 level: Circumferential disc bulging with hypertrophic facet joints is noted, causing compression to bilateral L5 traversing nerve roots.
L5/S1 level: Circumferential disc bulging with hypertrophic facet joints is noted, causing compression to left L5 exiting nerve root.
Conus medullaris is located at T12/L1 level. The visualized spinal cord shows normal signal intensity on all pulse sequences. No evidence of congenital spinal stenosis is seen. There is no abnormal paravertebral soft tissue.
IMPRESSIONS: Grade I L4/5 spondylolisthesis, with circumferential bulging of L4/5 intervertebral disc and hypertrophic facet joints, causing compression to bilateral L5 traversing nerve roots.
Circumferential bulging of L5/S1 intervertebral disc with hypertrophic facet joints, causing compression to left L5 exiting nerve root.
Degenerative change of the rest of the lumbar intervertebral discs as described.
Mild scoliosis of lumbar spine.
Few small round hyperintense lesions on T1W/T2W scattering at T12-L2 vertebral bodies, which can be suppressed by FS, possibly hemangioma.
